WHO WE HELP

Three services. Three different
problems. Same method.

DOMICILIARY CARE, MIDLANDS

The problem:  Hours doubled in eighteen months. The RM was doing rotas, safeguarding, supervisions and audits herself. Incident forms went in; nothing came back out.

What we did: Mock inspection found the gap CQC would find: no link between incidents and care plan reviews. Six-week implementation, loop set up, two senior carers trained to run monthly audits.

The result: Well-led evidenced with a live audit trail.
The RM spends half a day a week on governance,
not three.

NURSING HOME, SOUTH WEST

The problem: Requires Improvement in Safe. Nine falls in a quarter, four to the same two residents. The action plan was signed off but nothing had changed on the floor.

What we did. Clinical review of every fall against NICE guidance, a falls competency for all care staff, bed rail and sensor audit rebuilt. Falls now feed the risk register automatically.

The result. Falls halved the following quarter. Re-rated Good in Safe. Method rolled out to the provider's second home.

NEW REGISTRATION, PRIVATE CLINIC

The problem:  A new outpatient service with no governance history, a strong clinical team and a CQC registration to win in under four months.

What we did: Wrote the application with the nominated individual, built the policy set and the governance evidence CQC asks for, ran a mock registration interview with the RM.

The result: Registered first time, no requests for further information. Live on CareLearner the same week, so the first audit cycle started before the first patient.
